The politics of clean air: health, power and environmental justice

Clean air policy is not simply an environmental concern. It is a public-health decision shaped by transport systems, energy choices, industrial regulation, housing conditions and political power. Traffic emissions, factories, wildfires, polluting cooking fuels and construction dust create different forms of exposure, but the heaviest burden frequently falls on children, outdoor workers and communities with the fewest resources to avoid polluted places. Clean air therefore represents a basic condition for health, dignity and equal participation in society.
The scale of the problem is global. The World Health Organization (WHO) reports that 99 per cent of the world’s population breathes air exceeding its guideline limits. It estimates that the combined effects of ambient and household air pollution are associated with approximately 7 million premature deaths each year. These figures place air pollution among the most serious environmental threats to human health.
Fine particulate matter, commonly known as PM2.5 pollution, is especially dangerous because the particles are small enough to penetrate deep into the respiratory system and enter the bloodstream. Air pollution is associated with cardiovascular disease, stroke, respiratory illness, lung cancer and infections. Emerging evidence also points to possible effects on diabetes and children’s neurological development. The consequences extend beyond hospital admissions and mortality, affecting school attendance, labour productivity and household finances.
Yet pollution is not distributed evenly. Roads carrying heavy traffic are often routed through lower-income districts. Industrial plants, waste sites, warehouses and construction projects may be concentrated near communities with limited political influence. Informal workers can spend long hours beside congested roads or dusty worksites without adequate protection. Families unable to afford cleaner housing, air filtration or temporary relocation during pollution emergencies face fewer options than wealthier households.
This unequal exposure makes air pollution and inequality inseparable. Pollution may spread across administrative boundaries, but its risks are shaped by income, occupation, age, housing and access to health care. A citywide average can conceal severe pollution near a motorway, factory, port or construction corridor. National statistics can similarly obscure rural communities affected by seasonal burning, mining activity or household smoke.
Children are particularly vulnerable because their bodies are still developing and they breathe more air relative to their body weight than adults. Schools and playgrounds located near major roads or industrial areas may expose children throughout the day. Poor air quality can also deepen educational inequality when respiratory illness causes repeated absences or prevents children from participating fully in school life. The WHO identifies air pollution as a major environmental risk to children’s health.
Household pollution adds another dimension. Around 2.1 billion people remain exposed to harmful household smoke generated by open fires or simple stoves using fuels such as wood, charcoal, agricultural waste, coal or kerosene. The WHO estimates that household air pollution caused approximately 2.9 million deaths in 2021, including more than 309,000 deaths among children under five.
The issue cannot be reduced to individual cooking choices. Access to clean cooking energy depends on infrastructure, fuel prices, reliable electricity, housing design and public investment. A household may receive a cleaner stove but return to traditional fuels when gas or electricity becomes unaffordable or unreliable. Effective policy must therefore address energy poverty rather than assuming that information campaigns alone will change behaviour.
Outdoor air pollution is equally political. Governments decide fuel standards, public transport budgets, vehicle regulations, industrial permits and land-use rules. They determine whether polluters face meaningful penalties, whether residents receive timely warnings and whether air-quality targets reflect scientific guidance. Weak standards can allow harmful exposure to continue legally, while weak enforcement can make stronger laws ineffective.
Monitoring is one of the clearest tests of political commitment. Without reliable stations, transparent reporting and locally detailed data, communities cannot know what they are breathing or identify major sources. The United Nations Environment Programme (UNEP) reported that air-quality monitoring was not a legal requirement in 37 per cent of countries assessed in its 2021 review. UNEP also supports the GEMS/Air programme and other platforms intended to expand access to pollution data.
Low-cost sensors and citizen monitoring can help expose overlooked pollution hotspots, but they do not remove the state’s responsibility to maintain calibrated monitoring networks. Community-generated data may identify patterns and strengthen public scrutiny, while official systems remain essential for enforcement, health warnings and long-term comparison. Air-quality monitoring becomes meaningful only when information leads to action.
Wildfire smoke demonstrates the growing connection between air quality and climate policy. Fires can transport hazardous pollution across cities, regions and national borders. Climate change can increase the conditions that enable severe fire weather in many locations, while land management, agricultural burning and urban expansion influence local risk. Emergency alerts and temporary public-health measures are necessary, but they cannot replace climate mitigation, ecosystem management and resilient public services.
Several major air pollutants and greenhouse gases also come from shared sources, including fossil-fuel combustion in transport, electricity generation and industry. Measures such as cleaner public transport, energy efficiency, renewable power and better waste management can therefore produce both health and climate benefits. The World Bank has argued that air pollution and climate change should be addressed together, particularly in lower-income countries where health burdens and development pressures often intersect.
The economic case for action is substantial, although financial estimates cannot capture every loss. The World Bank estimated the global health damage associated with air pollution at US$8.1 trillion in 2019, equivalent to 6.1 per cent of global economic output. Such calculations include premature mortality and illness, but the lived costs also include unpaid care, interrupted education, reduced income and the stress of raising a family in an unhealthy environment.
Clean-air measures can nevertheless create political tensions. Restricting high-emission vehicles may improve urban health but impose costs on drivers who cannot afford replacements. Factory controls can protect nearby residents while affecting jobs when transitions are poorly managed. Bans on polluting fuels may harm low-income households unless affordable alternatives are available. Effective policy therefore requires a just transition, with financial support, accessible public transport, worker protections and realistic implementation schedules.
Responsibility must also extend beyond individual behaviour. Advice to stay indoors, wear protective equipment or avoid exercise during pollution episodes may reduce immediate exposure, but it transfers the burden to residents. Many people cannot work from home, change their route or purchase air-cleaning equipment. Outdoor workers, street vendors, delivery workers, construction workers and traffic officers may remain exposed because their income depends on being outside.
The politics of clean air is also the politics of accountability. Industries may dispute scientific findings, lobby against tighter limits or shift operations towards jurisdictions with weaker enforcement. Governments may prioritise visible infrastructure projects while neglecting less visible monitoring and inspection systems. Local authorities may lack the funding or legal power to regulate pollution produced outside their boundaries. Because polluted air crosses borders, national and regional cooperation is often required.
Civil society organisations, researchers and professional networks can help by analysing data, supporting affected communities and scrutinising public policy. Groups such as the Clean Air Fund support work on air pollution, health and climate, while the Health Effects Institute produces research on pollution exposure and health outcomes. Their work can complement public institutions, although independent monitoring and transparent funding remain important wherever evidence may influence regulation.
The issue is directly connected to SDG 3 (good health and well-being), particularly the target addressing deaths and illnesses caused by hazardous chemicals and air, water and soil pollution. It also relates to SDG 7 (affordable and clean energy) through access to cleaner household fuels, and SDG 11 (sustainable cities and communities), which includes monitoring urban particulate pollution. These connections matter because air quality is shaped across health, energy, housing, transport and urban governance rather than within one ministry alone.
Progress should ultimately be judged not only by national averages but by whether exposure falls in the most polluted communities. A cleaner city centre offers limited justice if industrial districts, roadside settlements and informal workplaces remain hazardous. Environmental justice requires governments to identify who is most exposed, involve affected communities in decisions and direct resources towards the places carrying the greatest burden.
Clean air cannot be secured through personal caution alone. It depends on enforceable standards, credible institutions, affordable energy, safer transport, responsible industry and accessible public information. Treating air as a shared public good changes the central question. The issue is no longer simply how much pollution exists, but who produces it, who benefits from the activities creating it and who is expected to bear the health consequences.
